Vertigo is a false sensation that you or your surroundings are spinning or moving, often accompanied by dizziness, imbalance and nausea. It is usually a symptom of an underlying problem in the inner ear or the balance (vestibular) system rather than a condition on its own.

Vertigo can come and go and is often triggered by head movements. Common signs include:

A spinning or whirling sensation

Common causes of vertigo

  • Benign paroxysmal positional vertigo (BPPV) — displaced inner-ear crystals
  • Vestibular neuritis or labyrinthitis — inner-ear inflammation, often after a viral infection
  • Meniere’s disease — fluid build-up in the inner ear
  • Middle or inner ear infections

Other contributing factors

  • Head or neck injury
  • Certain medications
  • Vestibular migraine
  • Problems with blood circulation

How vertigo is diagnosed

  • A detailed history and physical examination
  • Positional tests such as the Dix-Hallpike manoeuvre
  • Hearing tests (audiometry) and balance assessment
  • Imaging (MRI/CT) where needed to rule out other causes
Treatment

How vertigo is treated

Treatment depends on the cause. For BPPV, canalith repositioning manoeuvres (such as the Epley manoeuvre) are highly effective at moving the displaced inner-ear crystals and relieving symptoms.

Vestibular rehabilitation exercises help retrain your balance system, while medications can ease dizziness and nausea during acute attacks. Where vertigo is due to an underlying condition such as an infection or Meniere’s disease, treating that condition is essential.

Managing vertigo at home

  • Move slowly when standing up or turning your head
  • Sit or lie down as soon as you feel dizzy
  • Practise the balance exercises advised by your specialist
  • Reduce salt intake if you have Meniere’s disease
  • Avoid driving or heights during active vertigo

BPPV is usually treated with canalith repositioning manoeuvres such as the Epley manoeuvre, which move the displaced crystals in the inner ear and quickly relieve the spinning sensation.

Most vertigo is due to inner-ear problems and is not dangerous. But sudden severe vertigo with a severe headache, double vision, slurred speech, weakness or difficulty walking needs urgent medical attention.
